From the Poet's point of view his heart and mind are like the dying leaves of autumn. He uses it as an analogy of his depression and sorrow. What he is feeling is persistently dulling his mind and heart as though they were leaves withering from its branch. The reason for his sorrow is purposely left for personal identification and perception so one can relate to individual conditions that depress and dulls the senses.
