So I've seen posts About early twin cam 88's cam chain pads wearing prematurely. Does anyone know if they updated them ? I have a 2004 with 23,000 miles that I bought last year with 21,000 miles and figure that they probably haven't been checked.

Yes they were updated, 07 all models got the new tensioners.
06 Street Bob had the new tensioners
I would suggest gear drive
With your mileage you need to check your tensioners for wear for sure
You also need to check crank run out, this is a big problem on many Twin Cam engines leading to accelerated wear of all internal parts( this is VERY important if you are considering gear drive and ditching the cam tensioners)
If you don't already have a HD service manual I would get one before attempting any repairs, Good Luck and check back in
MIne were worn at 33000 on my 03. Ditched them for a gear drive. Friend had his making a lot of noise & had to replace on a trip. A year or so later they grenaded & had to have motor replaced. After switch to gear put another 70,000 on that 03 without a hiccup.
