Take it to an auto parts store and they should be able to match it up for you. Looks like one that any car would use. Rick

Been to the auto parts stores, ford. they have nothing close. it is an aftermarket heater provided by vintage air, don't know why they don't have a part?

sorry no help. that looks like a one off build that someone came up with to get the fan to work. TBMS must of subed that out to a small business. I think you might have to fix that one or find someone that knows how to make a replacement. Good Luck

I can't remember seeing one with that number of pins but judging by where its mounted and the circuit you need to be asking for the blower motor resistor.Ballast resistors are a different beast.
